Why an Enlarged Prostate Needs Treatment
An enlarged prostate needs treatment when it squeezes the urethra, the tube that carries urine out of the bladder, and makes passing urine slow, difficult or frequent. The condition is called Benign Prostatic Hyperplasia, or BPH. It is not cancer, and it becomes more common as men grow older.
You may need surgery when medicines no longer control your symptoms, or when the blockage starts to harm your bladder or kidneys. Common signs include:
- A weak or slow stream of urine
- Getting up several times at night to pass urine
- A sudden, urgent need to pass urine
- Feeling that your bladder has not emptied fully
- Repeated urine infections, bladder stones or blood in the urine
- Being unable to pass urine at all, which needs a catheter

Treatment Options for Enlarged Prostate: Green Light, HoLEP and TURP
Green Light Laser, HoLEP and TURP all remove the prostate tissue that blocks the urethra, and all three are performed through the urethra with no cut on the skin. They differ in how the tissue is removed, how long you stay in hospital and how large a prostate they can treat. Green Light Laser vaporises the blocking tissue with a high powered laser beam. Most men go home the same day with a catheter that is removed within 48 to 72 hours (BAUS patient information). In the GOLIATH trial, which treated 281 men across 29 hospitals in nine European countries, Green Light relieved symptoms as well as TURP over two years, and about five men in six had no complications in that time (Thomas and colleagues, European Urology, 2016).
HoLEP uses a holmium laser to separate the whole inner part of the prostate from its outer shell, and the tissue is then removed through the urethra. It works for prostates of every size, including very large glands that once needed open surgery. The catheter is usually removed the morning after surgery and most men go home within one to two days (Cambridge Urology Partnership). In a trial that followed men for about seven years, none of the men treated with HoLEP needed a second operation for their prostate, against three of the seventeen treated with TURP (Gilling and colleagues, BJU International, 2012).

TURP removes the blocking tissue in small pieces with an electrical loop. It has been performed for decades and suits prostates of 30 to 80 mL (European Association of Urology). Recovery and Life after Laser Prostate Surgery
Most men notice a stronger flow of urine almost immediately after laser prostate surgery, and are back to their usual activities within two to four weeks (NHS recovery advice). Full healing inside takes about six weeks, and the urgency and night time visits to the toilet can take about three months to settle.

- Rest for one to two weeks, and do not drive for at least one week
- Drink plenty of water to keep your urine clear
- Avoid heavy lifting and hard exercise for about six weeks
- A little blood in your urine can appear on and off for up to six weeks, which is normal; contact us at once if it becomes heavy or you cannot pass urine
Sex is usually possible again after three to four weeks. Many men find that little or no semen comes out when they ejaculate, because it flows back into the bladder instead. This does not harm you, and orgasm feels the same. If you plan to father children, kindly tell your urologist before surgery.

Air India flies Sydney to Delhi non-stop in about 12 and a half hours. Kindly check your travel insurance carefully, because standard policies often exclude trips taken for medical treatment. You are only three to four hours from Delhi or Mumbai, which makes laser surgery in India especially convenient. If your insurer covers treatment abroad, get written pre-authorisation before you fly, as some policies reduce a claim made without it. Your visa follows your passport, not your residence.
A Nigerian passport cannot use the e-Visa. You apply in person at the High Commission in Abuja or the Consulate General in Lagos, with the hospital invitation letter that we arrange for you. Flights connect through Doha, Cairo or Addis Ababa. Plan your payment early, because naira card limits fall well below a surgical package.
